When an approver declines an event, it would be beneficial for them to be required to enter a reason and/or note stating why the request was denied.

I definitely agree with this. I always have to call the person who declined it to find out why. It would be MUCH easier if they had to input a reason.
I think it would also be great to require notes for any time changes or edits to the requests etc.
