Ability to Manage Documents in MLS

I need to be able to do the following:

1. The upload section on the bottom of the request form to be customizable.

2. Name of Upload Document

3. Link to PDF of Upload Document Needed to be completed

4. How many files show

This comes into play for concussion release forms for athletic facilities, pools, and hold harmless agreements needed on a per-event basis.
